Street Outreach
A part of Housing Connect.
Street Outreach connects with people living outside and in other places not intended for housing. Outreach workers listen, build trust, discuss immediate needs, and help people connect with resources and Housing Connect. Outreach is voluntary and may take more than one conversation. A request helps the team understand where support may be needed; it does not guarantee an immediate visit, shelter, or housing placement.

When Street Outreach may be able to help
Street Outreach is for people living outside or in other places not intended for housing in Jasper and Newton Counties. You may request help for yourself, or you may share a concern about someone else.
I am helping someone else
Share only what you know. The person does not need to be photographed or approached for you to request supportive outreach.
How Street Outreach can help
Outreach workers focus on practical next steps and connections. Services depend on individual needs, program requirements, and what is available.
Immediate needs
Help with or connections to food, water, clothing, and shelter resources, depending on availability.
Housing Connect access
Help understanding and connecting with assessment steps.
Missing documents
Help working toward obtaining necessary documents.
Community services
Connections to health care, mental-health services, benefits, employment resources, and veterans’ services.
Continued engagement
Follow-up conversations and support connecting with next steps.

Use the secure form to request supportive outreach for yourself or someone else. Share only the information the team needs to understand the request and try to make contact.
Concerned about someone living outside?
You can request outreach when you are concerned that someone living outside may need help connecting with services. Share only what you know and the information needed to help staff make contact.
This form is for requesting supportive outreach, not requesting someone’s removal from a location.
Do not enter an encampment, photograph someone, or collect sensitive information just to complete the form.
A coordinated community response
Street Outreach is part of Housing Connect and can involve participating agencies, trained staff, and volunteers. The Homeless Coalition provides system governance, and Economic Security Corporation is the lead operating agency. Not every outreach worker is an ESC employee.
